    OBJECTIVES: This study validated the Japanese version of the Attention-Deficit/Hyperactivity Disorder-Rating Scale-5 (ADHD-RS-5) and the Disruptive Behavior Disorders Rating Scale. We extended the ADHD-RS-5 by adding the oppositional defiant disorder and conduct disorder subscales to compare the two rating scales psychometrically. METHODS: We examined the internal consistency, test-retest reliability, construct validity and criterion validity of the two rating scales in 135 Japanese outpatients aged 6-18 years. RESULTS: The internal consistency and test-retest reliability were good for all the subscales of the two rating scales except for the conduct disorder subscale of the ADHD-RS-5 extended. Good construct validity was revealed by expected correlational patterns between subscales from the two rating scales and the Children Behavior Checklist. The criterion validity was good for all the subscales of the two rating scales rated by parents, while teacher-ratings revealed substantially lower predictive ability for all the subscales. Agreement between parent- and teacher-ratings of the two rating scales was generally moderate and using predictive ratings alone of both ratings showed the best predictive ability among the integration methods examined. CONCLUSION: The two rating scales have sound psychometric properties and will aid in screening and severity assessment of externalizing disorders in Japanese clinical settings.

    筆者らは,大学初年次学生の協働学習に対する態度について,態度の3側面(認知・感情・行動)に着目した尺度,及び態度の両価性尺度を用いて調査を行ってきた.また,協働作業の指標の一つとして,編集履歴から作成プロセスを可視化するリビジョンマップを用いて協働ライティングにおける学生の貢献度や積極性を定量化する試みを続けている.本研究では,(1)態度の両価性と協働作業の指標との関連,(2)協働学習の前後での協働学習に対する態度変容と両価性との関連,(3)態度変容と協働作業の指標との関連について検討を行った.

    Abstract

    Background

    Mycophenolate mofetil (MMF), a prodrug of the immunosuppressive agent mycophenolic acid (MPA), is difficult to administer because of the pharmacokinetic complexity of MPA. Although dosage adjustment according to the 12-h area under the concentration–time curve (AUC0-12) is thought to be desirable, multiple blood samplings for AUC calculation may pose a clinical challenge. A limited sampling strategy (LSS) would provide a solution; however, little is known about MPA pharmacokinetics in lupus nephritis patients, especially in those with Asian backgrounds, or few, if any, LSSs are reported for them.

    Methods

    Thirty-four adult Japanese patients receiving MMF for lupus nephritis were examined retrospectively. MPA pharmacokinetics were investigated, and a PPK model was developed using Phoenix® NLME™ software. Single and double blood sampling strategies from Bayesian estimation using the PPK model and from multiple linear regression were compared. Tolerability was also evaluated.

    Results

    In the pharmacokinetic analysis, renal function and serum albumin had significant effects on dose-normalized AUC0-12; and serum albumin, concomitant proton pump inhibitor (PPI) and iron/magnesium oxide did on dose-normalized maximum concentration. As a PPK model, a two-compartment model was developed with a transit absorption model and first-order elimination, in which creatinine clearance and serum albumin were covariates for MPA clearance. The double sampling strategy at 1 and 4 h by multiple linear regression showed the best agreement with the observed AUC0-12 (r2 = 0.885). Of the single sampling strategies, the one at 6 h by Bayesian estimation performed best (r2 = 0.769). The tolerability evaluation showed that correlations were suggested for gastrointestinal involvement.

    Conclusions

    The present study developed the first PPK model of MPA for Japanese lupus nephritis patients. As for LSSs, a double sampling strategy at 1 and 4 h by multiple linear regression would work best; when only a single blood sampling is allowed, a strategy at 6 h by Bayesian estimation using the PPK model developed in this study would be best. The LSSs good enough for clinical use may facilitate safer, more effective, and individualized therapy.

    OBJECTIVES: This study was designed to examine the diagnostic performance of the social and communication disorders checklist (SCDC) and strength and difficulties questionnaire (SDQ) to detect autism spectrum conditions (ASC), along with the social responsiveness scale-second edition (SRS-2) as reference, in a psychiatry outpatient setting. METHODS: We translated the SCDC into Japanese since its Japanese version was unavailable. We examined its test-retest reliability as well as the internal consistency reliability and diagnostic performance of the three questionnaires among 41 Japanese psychiatric outpatients, using the best-estimate diagnosis of ASC based on the diagnostic interview for social and communication disorders, as a gold standard. RESULTS: The test-retest reliability was high for the SCDC. Although the internal consistency reliability was high for the SCDC and SRS-2, that was low for the prosocial and peer problem subscales of the SDQ. The performance of the SCDC, SDQ, and SRS-2 to detect ASC was moderate: the area under the ROC curve of 0.78, 0.78, and 0.84, respectively. CONCLUSIONS: Although questionnaires to detect ASC, including the three examined, generally have only moderate performance in this setting, these can be successfully applied to high-risk populations such as psychiatry outpatients, when multi-level rather than dichotomous likelihood ratios are used.

    本研究課題『社会的コンテクストの視点からの大学英語ライティング・タスク開発とオンライン提供』は、様々な学部等に所属する学習者が一つの教室内に混在するという多様性のある大学英語教育でのライティング指導に新たな展開をもたらすことを目的とする。具体的には、「社会的コンテクスト」という理論的視点を設定した上で、社会的コンテクストへの気づきを促すタスク群の開発およびタスク群と指導方法の体系化と教材化を進める。併せて、教材をオンラインで提供することで、得られた知見の社会への還元を目指す。2年目の2022年度は、以下の研究1-4のうち、研究2の後半と研究3を遂行し、研究4の準備を進めた。

    研究1. 「社会的コンテクストの視点からの大学英語ライティング教育の目的論の整理(2021年度前半):社会的コンテクストの役割に着目した文献研究を実施することで、大学英語ライティング教育の目的論という観点からのより精緻な理論的な整理を行った。
    研究2. 「社会的コンテクストへの気づきを促すタスク群の開発」(2021年度後半-2022年度前半):具体的なタスク開発を進めた。このタスク開発の際には、「なぜ書くのか」(Why)→「どのように書くのか」(How)→「何を書くのか」(What)という段階的な問いかけを、ヒントを与えつつ学習者自身に行わせることで、自ら適切な社会的コンテクストへの気づきを得ることを補助することを目指した。これを実際に授業実践を行う中で進めた。
    研究3. 「タスク群と指導方法の体系化と教材化」(2022年度前半-2022年度後半):得られたタスク群をもとにして、指導しやすいように、それぞれのタスクに難易度や指導順序等の説明情報を付与することで体系化することを試みた。この段階での中間報告を研究ノートとして出版した。

    本研究の目的は、医学生を対象とした、英語医療面接向上のためのVR(バーチャルリアリティ)・ロボット教材を作成し、その効果を検討することである。VR教材では、学習者である医学生が医師役となり、VRゴーグルを装着し、病室の場面で、患者と医療面接の会話を行い、ロールプレイを行うことができる。ロボット教材は、ロボットが患者役となり、医学生が医師役となることで、英語医療面接の練習が可能である。ロボット教材では、英語医療面接の会話練習を行った後に、要点を確認するためのクイズを数問提示した。
    研究初年度から、英語医療面接用のスクリプトの作成や、VR・ロボットの英語医療面接教材の試作と少人数での試行などを実施してきたが、地域の基幹病院を有する医科大学としてコロナ感染症対策が徹底されていたために、この3年間は、実際に医学生を対象とした調査を行うことが困難であった。そこで、当該年度の前半では、患者とのラポール(信頼関係)を育む医学英語カリキュラムの検討を行い、後半では、コロナ感染者数が比較的少なかった頃に、研修医を対象として、VRやロボットで英語医療面接を行う教材を試行する予備調査を実施した。その後、医学部1年生を対象として、VRやロボットの英語医療面接教材を試行する本調査を実施した。加えて、コロナ感染症対策のために、学生はマスク生活が続き、声を出す機会が減っていたために、英語を話す時にも声が十分に出ない傾向が見られた。そこで、英語医療面接の時に、医師が、患者に聞こえやすい声で英語を話すことができるように、英語発音トレーニングにおけるヨガの効果に関する研究を行った。

    本研究では、Googleドキュメントなどのクラウドを使った協働ライティングを対象とした研究を行っている。これまでの研究成果として、ライティングプロセスを可視化するリビジョンマップを改良し、2種類の改良型リビジョンマップ(セクション別、編集者別)を作成して分析を行ってきた。
    前年度は、リビジョンマップの適用例として編集者別の改良型リビジョンマップからレポートに関する貢献度が低い編集者(フリーライダー)がいるレポートの分析を行った。今年度はこのマップから得られるデータを利用して、いくつかの指標を作成した。指標の1つ「レポートのセクション重複率」は、レポートを構成する各セクションを編集する人数が増えると数値が大きくなり、個々の編集者が互いに干渉せずに独立して各セクションを編集すると小さくなるため、レポート作成における協働状況の指標になると考えられる。別の指標として「入力セクション比率」は、レポートを構成する全セクション数に対する個々の編集者の入力したセクション数の割合を示していて、レポートに対する個人の貢献度を示す指標になると考えられる。
    これらを含めた指標を用いて、コロナ禍でZOOMによるオンライン対話を中心とした状況で作成した協働レポートと、コロナ禍前の対面による対話を通じて作成した通常の協働レポートについて比較を行った。その結果、コロナ禍での協働レポートではフリーライダーに相当する編集者が減少したことが明らかになったが、全体としては大きな差異はみられず、クラウドを用いた協働ライティングは、オンライン中心の制限された状況でもある程度有効に機能したことが示唆された。

    In the midst of the growing demand for increased international competitiveness in various academic fields, the ability to write about research results in English is a key factor in each discipline. However, there are various constraints and limitations to the teaching of English writing for specific disciplines in the classroom. Therefore, we have developed and been using an English academic writing support tool as a supplement to their instruction in and out of the classroom. The present study aimed to improve and extend this tool, and to develop teaching methods and materials that utilize the tool. Specifically, we (1) added more disciplines and ragged research papers, (2) improved the tool, and (3) used the tool in and out of the classroom. The results of our research have been published in papers and at international conferences.

    We have developed a rubric to assess the performance of first- and second-year Japanese medical students taking patient histories in English. Our rubric consists of the following dimensions: 1. Spoken English Proficiency (SEP) to evaluate fluency and accuracy in speaking English; 2. Communication and Interpersonal Skills (CIS) to evaluate empathy for building rapport with patients; and 3. Sequence of Questions (SOQ) to evaluate responsiveness for digging into a patient's story. The first two sections were based on scoring subcomponents used in the USMLE Step 2 Clinical Skills, and the third section was especially designed for the purpose of assessing pre-clinical medical students. SOQ only focuses on data-gathering skills, without requiring mature clinical knowledge that is necessary to narrow down a possible diagnosis.

    We have investigated what reference sources first year college students rely on for report writing. Noticing that the students very often copied and pasted from the Internet, we have analyzed copied and pasted parts of texts for information credibility, and found that the students relied greatly on online information of little credibility. We have further investigated the revision histories available on the cloud system, finding that the students tended to use information of little credibility in improper ways, including plagiarizing, whereas they were likely to properly cite information from more credible sources. Supposing that improved reference credibility could help prevent plagiarism, we have intervened writing process by adding a step where the students themselves examined reference resources. The analysis of the reference lists, however, revealed that the step alone did not work well enough for improvement in reference credibility.

    Writing practice done in language classes may lack social context, resulting in little attention to the purpose or audience of writing. Our project aims to develop writing tasks that allow second language (English) learners to practice writing to achieve an objective with good awareness of audience. We have developed a pilot task where learners are asked why to write, how to write, and what to write in a consistent manner so that they may raise their awareness of the social factors involved in writing.

    In our tutorial education, students were given a task of writing a report online, using Google Docs. In our first study, by tracing the work history obtained from online report files, we analyzed their writing process, focusing on the process of copy-and-paste plagiarism. An analysis of the change log of written reports enabled us to visualize the students’ writing process. Sudden increase in total letter count demonstrated the possibility of plagiarism. In our second study, each report was scanned with plagiarism detection software, and its work history was analyzed. As a result, a positive correlation was found between copied-and-pasted text percentage in a submitted report, and copy-and-paste frequency in its writing process. Our third study examined unnatural null-deletion typing behavior. Instead of copying a whole passage from the Internet, every part of the idea was taken and typed continuously with minor modification, without a rethinking process.

    When a particular rubric is shared by different users, understanding and interpretation of the rubric may vary depending on the users, which could lead to reliability or validity problems. One of the solutions is to supplement a rubric with well-articulated interpretations and relevant writing samples. We have conducted the studies to determine such interpretations and writing samples supplementing an original L2 (English) essay/paragraph writing rubric, which the research group have developed for classroom use in Japanese higher education. On the basis of the results, we have prepared the interpretations and writing samples. We have also implemented a teaching plan where students use the rubric for peer assessment as well as self-assessment. The questionnaire to the students showed that the rubric helped them identify aspects needing improvement.

    We examined an EFL writing rubric, which we developed for Japanese college English education, in terms of reliability and validity. As a result, we confirmed its reliability to some extent, but the validity was weak in a few dimensions. Therefore, we revised the rubric to improve the validity. We also changed the rubric so that it has several defining descriptions under each dimension and no preset scales. It enabled raters to set a score range freely according to the level of subjects. In addition to the use of the rubric for evaluation, we used it as a revision tool in classroom. The students who used it as a revision tool thought it effective in two dimensions :"idea and development"and"organization,"and actually their post-essays improved in the general assessment and assessment in each dimension.
